Black ghost knife with a hole?!

Hi I have had Knifefish for years and specially black ghost knives. I recently noticed a hole in one's body near his stomach I have no clue what it is. I will attach a photo.

What are u feeding it? Do u have an exposed heater? Water parameters?
 
Make sure water is clean and adds salt but for half the tanks size. Is the heater covered? In case it's the heater keep horizontal and at the top of the tank.

The nitrate is really high. And it's because my sump likes to collect poop and all underneath of it and in the over flow sides and it's hard to clean. I'm going to do that tomorrow but for now I'm going to get some stuff to help the nitrate until then.
 
Get some rid-x as well. Or look into it as part maintenance. Get those wc's with prime in as it'll help to heal up ur ghost knife in no time.

I only use prime for wc and I have to fix my filter to help my nitrates.
