My little guy has been like this for about 4 days. He is also breathing heavily and a little darker than usual but he might be showing his breeding colors. Only other tankmates are a female green texas and 2 platies who are healthy with none of his symptoms.

Could this be from him eating too many bloodworms a few days before his stomach got bloated?

Once I enticed he was bloated I stopped feeding him for the first 2 days. On the 3rd day i fed him 2 1/2 peas. Then today I fed him some pellets soaked in water with Epsom salt just in case he has internal parasites.

bloodworms = constipation. I never feed my big flowerhorns high protein foods as they tend to get all types of internal issues.

Should I just feed peas or pellets soaked in epsom salt?

u dont need to feed epsom salt directly. just apply it to water. the peas are good for fiber. you must cook them just barely before feeding and peel the shell also. Stop feeding which you already have done. Fish can go pretty long without eating.

umm you can fast a fish for a week easily and I'd think it should go away by then. If not, make sure you're keeping you water clean and start looking into treating for dropsy. Has it gone down at all or is it still bad? Make sure you keep dropping salt in. Are his scales beginning to protrude like a pine cone? Or are the eyes bulging?

If you're in fact seeing those types of symptoms, get a hold of some Metronidazole. You will want to dilute it in water 1% solution, then soak food in it, and freeze or put it in fridge for a few hours. Only feed 1% of the fishes weight on a daily basis. Do not misuse it or it can result in death of your fish. You can also use Metronidazole directly to water at 25-95 mg/gallon.

You can also use clout, but that is a add to water type medication. If your tank is big, probably want to set up a hospital tank.

The water is clean but ill start doing larger water changes every 2-3 days until he gets better. His scales and eyes are normal. He still swims around and is definitely hungry.

2 days ago I started feeding him pellets soaked in epsom salt then did a 50% water change and added epsom salt to the water. I started feeding the pellets with epsom salt because RD posted that its a good safe cure for spironucleus which might be the cause of his swollen belly. I've seen him poop a little but its not clear or stringy. His belly looks more bloated now though.

hex has a very distinct symptoms. not to mention, they do not like eating and will be listless resulting in emaciating body form (thinning). But that's not to say they can't become bloated since it is a parasite that infects the intestines. I'm pretty sure by what you're saying he is constipated big time. Especially since you specifically said you've seen him poop a little. Pooping a little for this fish breed is not possible, they are messy guys. Stop feeding anything for a week and just keep water clean. Keep an eye on him for any signs or symptoms and don't feel bad when he swims up to the glass for food. keep adding salt every water change also.

I took your advice and stopped feeding him yesterday. But when I got home today I saw a tiny white thing sticking out of his butt. It wasnt translucent though but I'm worried what it could be. I went to eat dinner and when I went back it was gone.

I think I may move him to bare bottom 10 gallon to monitor him better.
