I'd say the high nitrates over time just got to him. Purigen (I use 5x the recommended amount) works well to keep nitrates down, also pothos do a great job. The two together keep my heavily overstocked 225g in the 15-20 nitrate range even if I don't do a WC for over two weeks.
Most importantly, a 50% WC weekly. More if you feed heavily or are overstocked.
