Broko with blood red, swollen anus

Has been happening for over a month, hes been a bit skiddish and lethargic. His anus is extremely red, blood coloured, and visibly swollen. No red worms hanging out of it as of yet. Still eats. Sorry for bad pics, just moved him to quarantine tank 15 minutes ago.

Could be constipated. Stop feeding so he can flush out his system

It's safe to starve a 6 - 7 inch pbass for a week that may have other health issues? I will admit it's possible about the gravel though for some reason I am skeptical as sometimes it's super red and swollen, sometimes not as much. Any other suggestions for treatment? i dosed prazi once a couple weeks ago and it seemed to reduce it but like i said it comes and goes sometimes so it could be a coincidence. been going on for over a month. wondering if theres a medication i can treat that would cover most possible causes for the symptoms we see.
 
A LFS guy suggested catching him, and dabbing some peroxide on it with a q-tip as he thinks without worms hanging from the bottom its likely to be an infection, especially since it gets better and worse periodically. Does anyone have any thoughts on this?
 
I put peroxide on it, it fizzed a bit but not much. It is now not as swollen, but a steady shade of pink and still definitely a bit swollen. Also he has not been crapping that much, and when he does it is stringy, often white. Anyone have any suggestions?
 
thank you for the post, but there are absolutely not any worms hanging out. not a single one, not the begining of one. not yet anyway, and it has been over 2 months. plus he is still eating and showing decent colours. i hope it isnt that though.
 
i have been adding salt, not epsom salt though. would you know how to dose or how much? tank is 48x18x18 (75 gal?)
