Bubbles on the surface..

Adding a powerhead to get a circular surface flow will help. The bubbles are caused by a protien film trapping air.
 
it's not only form a protein film... my what levels are great... and there is not film on my tank... but originally with by fizz stone... the bubble were small... smaller bubbles have a higher strength then larger ones... so they hold together and they would build up... so w.e. is making the bubbles may sometimes be the problem too

also are there any filter media that will remove this?

i think i got a piece of shrimp sucked into the filter, but could that possibly be a cause?

Magic. Lots and lots of magic. I hear that a super fine stone and a LOT of air will help it work, but success is pretty limited. Water changes are cheap, easy and do the job MUCH better (gotta love 'em).
