sorry to hear Bruce, it must be only a slight case since all your recent pics of the big guy dosent really show it. the only thing i can think off is to increase your WC schedule and keep varying that diet.. best of luck mate and keep us updated..

I don't understand why your nitrates are so high. That is probably the cause of the HITH. Try getting a new nitrate test kit to see if it is accurate. Also test your tap water. Mine never exceed 20ppm so I don't understand how yours can get so high at 250ppm.
 
What is your PH?

I have seen cichla get marks that look very similar to HITH when the PH gets too low...
 
My water quality is super clear and always has been, im way overfiltering and using a UV steriliser. My nitrates are high around 200-250ppm. But i do a big weekly 50% waterchanges as im only alloud to do it once per week. I feed only the best, i use Hikari and good meaty feed. I love him to bits. Its notoverstocked at all either...... Any help?

Dude your Nitrates are way to high IMO
You need to get them around 20 no more than 40 ppm.
How often do you feed and how much ? Do you gravel vac when doing WC ?
I hope he gets well soon Bruce he is such a awsome fish.
 
muscles and cockles can be very dirty Bruce I would just feed them prawn and pellet for a while .
