CLown loach desease

You need to get a nitrate test. Not the paper strips but one that uses test tubes and drops. I saw a picture of a Clown Loach with Hole-In-The-Head (HITH or HLLE) once and it looks exactly like that. I'm looking for the picture on Google right now. The biggest cause of HITH isn't Hexamite like some believe but high nitrates. Which could be defined as nitrates over 20 ppm in fish that are sensitve to water quality. (Which Loaches are).

It's not the picture I was thinking of but it'll give you something to compare your little guy with. HITH is pretty controversial as far as the causes as nobody really knows for sure.
 
definitely looks like hole in the head disease, go to your LFS and get some meds, and do daily water changes of 15-20%, your water parameters have got to be off, prob high nitrates
