I guess I'm asking deeper then that. Is it poor diet or poor water condition that's believed to be the primary cause of HITH?

Infact both, bad diet causes lateral line disease which is basically HITH and bad water quality lowers the immune system of the cichlid and allows the parasites to latch onto the cichlid. I usually find that people who poison their oscars with low nutrition/high thiamine content feeder goldfish have oscars with HITH. The downside of frozen feeders is you cannot "gutload" it unless done before being frozen. But even then you can add vitamin supplements. PLus I'm sure he fed his bala and he froze it killing the parasites so It's fine, plus it's not like his staple is frozen balas right? XD be pretty expensive.
