I add a piece of driftwood every 6 months to keep tannins up and add 5 Indian almond leaves I should probably add more

Your water is very much the same as mine was in Milwaukee, and to keep my nitrates down to between 2-5 (my goal), I had to do every other day 30-40% water changes, and in summer when I got rain water, every day, with the tannin infused water. Seems like a lot, but its basically the price you pay for keeping these seasonal black water species healthy long term.
The Central American, and African hard water species, were much less prone to those type ailments, even when I got lazy and slacked off.
