I woke up this morning and saw that my delhezi had this white patch on his head. it wasnt there last night and i caught it right away. What is it and what should i do?

I can't tell you if that's columnaris or not. One treatment for columnaris is Kanaplex with Furan-2, along with 1 tablespoon of salt per 10 gallons, temperature 75F.

If you decide to treat, I know that Seachem Kanaplex and Seachem Paraguard is safe on plants and a fully cycled plant.

i still have to remove the carbon for it right. and yah my tank is very very heavily planted. is melafix also plant safe?

I would remove the carbon for any medication treatment. Salt would affect your plants.

Melafix won't work against columnaris. It's rather weak.
 
Decor in the tank? Looks like a scrape. I’d watch for a short bit with some water changes. Just IMO.

there used to be nice manzaneta branches everywhere but i removed them a long time ago, its litereally smooth river stones, sand, and plants. im assuming my other bichirs got him, but hes 16 inches and the others are all like 12 inches or less, so they had to have had some balls messing with him.
 
would you recommend melafix if its alongside pimafix tho?
No as it won’t help. I’d give it time and let it eat. Watch and it should get better. Never seen a Polypterus with columnaris. Any fish can get it. I agree with rocksor as you can’t really say it’s not columnaris.

tank you guys so much. i just went to look at her a bit more and its reflective and flat, so I think you were right, tis but a scratch. i remember last night on accident i put all the pellets in one corner of the tank, so i think one of my other 6 must have bit him on accident.
