Whether it be letting the tank overflow or forgettig to close the hood or even overstocking, post it.

So far:
No hood for 12gal, 8 fish total jumped, 5 saved
Put an oscar in a 12gal ( beginner mistake )
Put untreated water into a almost cycled tank ( ruined the beneficial bacteria  )
The list will continue but thats all i can remember

for me it would be

a bit of ich showed in a 5 side corner tank, rather than get some advice from others on what they thought the g size was, I treated it according to the g size i thought/told it was and ended up dosing a 45g as if it was a 60g little wonder all the fish died!!!! It would seem that melafix has the ability to suffocate fish when overdosed. along with the ich medicine

First I accidentally forgot the hood wasnt on right and while I was doing a wc I bumped it and it fell in! It was unlpugged though. The othere mistake that was I treated beunos aries tetras with ich meds that it clearly stated on the bottle not for tetras. Oops.

Not my mistake, but my husbands. A 75 gallon tank sprung a leak so it was emptied, cleaned and stripped of old silicone and re-siliconed.....

He decided to speed up the drying time so that all the fish that were sitting in a cramped 30 gallon holding tank could go home faster. He used a heat gun on the silicone! (A heat gun is like a hairdryer only it blows air hot enough to be used for stripping paint off of wood furniture.) He broke the glass. Big shock there!

For me was when I accidentally knocked a light into the tank, I by reflex reached into the tank and fished it out without thinking until after I pulled it out. Lucky I didn't get electrocuted (big dumby).

bought a rtc put it in a 10gal didnt research it and it got 10inch from 2 in about 2months

my mom wonderd wy it got so big so fast so i looked and found out it got huge and the tank size is 400gal +
i sold that sucker back to the lfs the next daY A GOT 75 $

KInda a mistake but it worked out in a 60$ payout :)

Hmm. Mine happened tonight. I'm cleaning all my smaller tanks i have laying around. I have (had) this really old 29 gal with a mirrored background from like the 80's. I finished cleaning it and went down my steps. Well the way I was holding its was stupid. one hand on the trim one on the side. Needless to say the 20+ year old plastic trim snapped on the last step. Busted the tank and damn near lost my foot.
