fish is real, just that its taken upclose/zoomed, just compare the lipgrip he is using to the fish mouth/head those who have seen a lipgrip can roughtly estimate it

although it looks kind of photoshopped, it could easily be real; based on perspective.

P.S that means it's not actually massive like in the pic, if he held it close to his body you'd see a better relative size to the guy in the pic
