Taping all the joints of the box is a very good suggestion. As well as duct taping the bag and taping the heat packs to the lid.

Just snapped a pic of her this morning. You can see the tips of her dorsal fin is frayed as well as her pelvic fins. It does look better than yesterday though and she's not clamped anymore. She's very interactive with me and seems to be feeling good. She even flared at me just a bit lol.

Yikes, didn't realize her dorsal was that bad from the first pic you posted. Ya, she's always been a really good eater.

Would it be beneficial to place some bio media in the bag or would the lack of oxygen just cause it to foul the water ?
 
I would say no. Some people use a small piece of Poly-Filter by Poly Bio-Marine which is supposed to absorb ammonia.
