Fresh Water stone fish mysterious death?

So I found him dead in my tank today, covered in what looks like velvet but he was fine when I went to sleep. This is the second time this has happened what am I doing wrong?
 
post up ur water tests. ph an all that stuff. also temp. an what setup u got going, also pics can help
 
Yup...even though they are labeled as "Freshwater" they really are Brackish water and do better with some salt.
 
Agree with the comments that the stonefish requires marine type environment with high salinity. Many have kept them in FW but found they just could not survive for long.
 
I realize that they are brackish but I was talking to some other keepers that have had theirs in fresh for some time without problems but for some reason I cant keep them alive in it. I did add some aquarium salt when I put him in but it was more for stress then salinity

40gal breeder some driftwood and granite along with live plants
I didnt test the water but everything should be fine the tank has been up for years with other inhabitants that have never had issues temp is about 81

This is not the same tank I lost the last one in tho, I lost the last one in a 75 about 2 years ago
 
Sometime ago, some colleagues showed pics of their stone fish and I really wanted to keep this fish, but one by one, they died, and without reason. The conclusion was that they can't be kept in FW. But if your colleagues have kept them, perhaps find out how they do it. I remember vaguely there could be a feeding problem as they don't move a lot, but to get around this, those small catfish feeders can be deployed as they swim right into the fish's mouth. I really wanted to keep one of these coz they look like the Sesame Street Oscar, really comical looking.
 
There are a handful of very similar looking fish, all that are lablled 'freshwater stonefish'. Perhaps your friends just got lucky and actually got the real freshwater ones.

Bear in mind also that a lot of the true freshwater species prefer to be kept in harder, alkaline water.

First of all, what species of lionfish?
There are around 69 species of lionfish/stonefish/toadfish, from full salt to full freshwater.
All are predatory and will wait for food to swim by, you need to make sure it eats, either by the use of feeding tongs or feeding rod with which to place food in front of the lion to stimulate it into feeding.
Not all lion/stonefish require salt(that is a myth)For example: Thalassophryne amazonica, Notesthes robusta.
Many freshwater stonefish live in estuaries. This means when the tide goes out they live in freshwater and when the tide comes in, they live in brackish.
