How’s your water quality
I’ve always found that a little more water changes per week helps repair fins and cuts
Is there anything else in the tank that may be picking on him ?
Love a green phantom, here’s a photo of mine

I've had him like this since I've gotten him,all my other fish in the tank including the blue phantom I got with him in the same shipment are doing amazing he his the king pleco in the tank as he's the biggest fish in the tank as my peacock bass are still smaller his tank mates included 1 blue phantom 1 scarlet pleco 2 peacock bass 1 bicher sorry for spelling and bristle nose plecos I've had other plecos have the same issue and been able to grow them back but not this guy

I’m not 100% sure if aquarium salt would help in any way you might need to wait for a pleco expert but I always do 2 10% water changes per week and maybe a 40% change on the 3rd week when I ha e these issues.

Yea its weird for sure I've had him out in hospital tank by him self as well nothing I know I shouldn't worry till he stops eating but thats never been the case more less curious

Judging by the shape of that split, and if it came to you that way, I'm thinking maybe a minor cosmetic defect rather than an injury to the dorsal. Happens sometimes, in various species you can get a few like that as they grow out from fry-- what a breeder does from there depends on the breeder, the species, the overall look of the fish, etc.
