Do you guys think my GT has a HiTH disease ? If so is quarantine / treatment required ? The rest of the fish (BP) tank are doing fine. GT is still eating well and being terrotorial as usual no unusual behavior.

Could it be a mere wound and it will self heal ?

I'm using Easy-Life Voogle everyday to boost immunity.

Water parameters are fine. Refreshing every 2 weeks with. 0% ammonia, 0% nitrite.

To me looks like sensory pits and an injury on top butm it could very well be HITH I have seen HITH in two different forms
1. lots of small pits that slowly grow
2. several large pits
thats just what I have seen. I would bump up water changes to maybe 50-80% a week or do what your doing now twice a week
what size tank? your water change scheduled seems a little light. also 50 Nitrate is quite high usually aim to stay below 20 and 10 being ideal so definitely bump up the water changes

It looks like it but not totally sure. I suggest you do larger percentage water changes. Your Nitrate using a liquid test kit would read 50 ppm but that is high.

I see 2 pits, hence I believe it's HITH. One is the big one, and the other is just above the eye. HITH will affect fine with HITH. Once HITH gets infected with a bacterial infection, then you will see a change in behavior.

Metronidazole and increase of water changes to weekly (50%) will be required at a minimum.
 
Not to be difficult, but to me the spot on the top of the head looks like a ding/mild gash. It does appear to be starting to fungus a little, so for me I'd treat with clean water and either a mild dose of Melafix or meth blue (which I've done in mature tanks/mature filters with no problems, but might be better in a separate tank) and see if it improves. That the gash might be fungusing and another pit is possibly affected could be telling you to do bigger and/or more frequent water changes and/or don't let filters build up too much gunk. I'm speaking generally, not questioning your tank maintenance. Ime in some tanks it's certainly possible to do water changes every other week and maintain water quality-- low bio-load to tank size, for example, or some specialized setups, but imo I'd interpret what your seeing as telling me to change more water.

Even with a lot of filtration and clean looking water you don't see the bio-compounds or bacteria load in the water, but other things can hint at them. Imo great filtration can sometimes make water look good when it's not as biologically pristine as it appears to the eye, I've learned that over the years from the times when my water still looked great but fish were beginning to flash a little and it turned out a filter needed cleaning. I've come to the conclusion great filtration sometimes masks less than perfect water.

I've only ever had a single, relatively mild case of a sensory pit issue, potentially HITH, and by catching it early I successfully treated it in a short time with meth blue.
 
In total there are 5 holes, 3 small ones, and 2 big ones. I think it is HITH, because how could it be a gash? Never heard of pit fungus. kno4te kno4te , what is your opinion?

I only see 4 holes. I see 2 small holes as being natural, one for the nasal passage and the other for the sensory pit (above the nasal passage). I'm not sure where 3rd small hole/2nd largest hole is?
 
When I see a reading of 50 mgL nitrate, I'm not surprised to see HLLE, or many other usually benign bacteria becoming infectious.
These fish are endemic to waters that usually have nitrate reading less than 5 ppm.
In my opinion, water changes of only 30% every 2 weeks are the source of the problem.
To me, two 30%water changes per week would not be too much, and might eliminate the problem, as they would bring down nitrate to a more healthy level.
 
I had hole in the head on my salvini male and nothing helped it at all, no water changes or medication would heal it up.
I tried 3 different top of the range meds and all the hope in the world but nothing healed it up

this is a pain in the ar*e disease
