Your set up is bad thats why. Bad water and a system that creates bad water.

Try bigger tank/more water, More media, and more flow/movement in the water.

For now ypu needcto add medication for cloudy eyes or he will start getting pop eyes

Try bigger tank/more water, More media, and more flow/movement in the water. And change filter pads.

make those changes; then add medication for cloudy eyes or he will start getting pop eyes next , and then blind after.
