It looks like advanced hole in head, with secondary infections. My heart goes out to you, severums are very personable fish, and this is VERY difficult to treat. By the time you see the white goo coming out of their head, the infection is pretty far advanced (the protozoal parasites responsible are present in low numbers in the guts of MOST fish, and it turns into an infection when their immune systems are weak). I had a similar experience, and had to BUST MY BUTT for three weeks with water changes, force feeding, baths, medicated food, you name it to save the fish.
