It's hard to figure out your problem when your post is blurry. What do you feed your fish? At the rate you fed thrice a day, your water changes should have been adjusted to daily especially when you consider the amount of poo they excrete and tank size. What is the temperature?

Its a deformed discus. I forget the proper name but its a deformed gill thus inferior. I think Wattley said from Asian overbreeeding?? I have one too but his is only on 1 side, yours to probably, so I only photo him from that side. Its not a cull, its a beaut!

I would say that it could be really stressed, or the gills are inflamed with a infection. I would be worried too since you say the skin is coming off. can you define that? when you say coming off are you talking about like it coming off as if there where baby discus feeding on the? can't think of the word but the parents produce it and the baby discus get feed, or you talking about pealing off like it's got a sun burn?
