I've head of hole in the head, but what about hole in the face? I realize I shoulda done something a lot sooner, but he had at first what seemed like a stretch mark from eating pellets that were too big for him. For the longest time it was there and I thought it would go away. It didn't. And now it is sort of deteriorating and you can see to his mouth through it. I feel extremely bad and want to help him. Any ideas? Please hurry with any ideas, causes or solutions.

Oh my god thats rough man. I would isolate him in another tank and keep your water quality PRISTINE, like w/c as much as possible without stressing him, salt 1 tbsp per 5 gals, and pull the carbon from your filter. Good luck with it.
It's been like this for some time and none of the other fish are affected. I do water changes normally...

that looks really bad. you need to get him out and put him in a hospital tank. i would buy some Jungle brand "tank buddies" its a anti fungal but it works for gram negative rod bacteria--i have used it before for sores and it worked. he needs pristine water, i would also add aquarium salt too.
i dont know if that will heal-but i would try. best of luck to the little guy-it looks really painful. if you dont get him out, he runs the risk of that getting a fungal or bacterial infection from being overstressed, his immune system compromised, and not having constant waterchanges.
