Yeah, His head was like bleeding which made the water look blurry, I did water changes 2 times a month.

Thats why he died, you need a bare minimum of 1 a week with 25%+ new water every time!!!
I thought the temp should be lower, and no salt for this particular situation. Just water changes.(??)

I agree with the poster above, water changes are essential. FH's produce a ton of waste and the nitrate level can get out of control without water changes which can lead to health problems. I do 50% once a week on my Green Terror's tank. The Nitrate rarely gets past 40ppm.
