Yikes. Tumor maybe? I really wish I could help, but I’ve never seen anything like this before.
There are koi forums you could post this on to get specialized help. I’m not sure if it’s frowned upon to name it here, but just google koi forums and it should pop up. It’s the very first one that comes up for me.
