it's probably from some of the new plants I added to the tank.
don't really over feed my rhom but I do leave the food in there longer than I should because I'm trying to get him start eating prepared food. maybe that's why there're so many of them now.

Should I also change my filter medias as well? what about bio media? can I just wash them under running water?

General cause should/would be over feeding and or lack of tank maintence...

just had the fish in that tank for a little less than 1 week so I haven't done any wc yet.

I wouldn't change it, I'd just wash it out. Try to wash it in tank water in a bucket after a water change if possible.
Only leave the food you offer in the tank for a few hours max and then remove it. To be honest, I'd cut out feeding all together for a week or two to help square away the tank. It will only help with getting your piranha to accept the prepared foods your offering.

Cut out feeding for a week or two sounds a good idea.
as for washing filter media in tank water, won't the tank water still fill with those worms?
what about just do wc for a week or two first and see if the worms decrease then wash the filter medias in a "cleaner" tank water? sounds reasonable?

big thing is taking the food element away from the worms with wc's and no excess of uneaten food, they die (which they do pretty quickly) and getting them out with water changes.

all the parameters are good, the tank has been running since summer with two LMB in it. most likely got those worms from new plants I put in.:(
with me leaving food in there longer than usually to train the rhom to eat prepared food caused this worm out break.
 
WOW. I just discovered the same problem in MY tank yesterday! I have been searching for an answer for this since yesterday. The only thing that bothers me is I have NO live plants in my tank. However, I have been experimenting with my pleco offering vegetable matter in the form of blanched food for a few weeks and I have been feeding frozen bloodworms to my fish and frog. I did a 50% water change about 3 weeks ago and introduced nothing new in the tank since then except for some butternut squash and some green beans, and I removed that in a day or so when the pleco showed no interest. I will do a water parameter test tonite.
