Does look a bit like Hole in the head disease. If diet consists of feeders you may consider changing to a pellet or shrimp stuffed w pellet. In any case I would salt up to decrease risk of secondary bacterial infection. Maintain excellent water quality, check PH and Ammonia and perform weekly water changes. Consider a round of antibiotics impregnated food. Good luck.
