HELP! Tiger Oscar sick, please help me figure out what it is!

Please help, I have a sick tiger oscar, the tank is 90 gallons in size, I have him living with 3 baby green terrors and 3 oscars in similar size, 2 plecos and 2 crayfish (1 blue 1 reddish). Here are the most non blurry shots i could get of him, I noticed a white spot under his eye, I kept him isolated and treated him with furan 2 for a week, that didnt seem to do anything, I also tried a little of fungus cure, but it dosent look like fungus because its inot extruding, infact it looks white and kind of scooped in. His behavior has been as active as usual, infact i felt bad for isolating him just for how good he is. He's now back in the 90 gallon, I DONT KNOW WHAT HE HAS! Please help if anyone has seen this problem before, let me know, I dont want him to die, his size is about 4 to 5 inches. he loves to jump up for food and sometimes he bops his head on the top, I dont encourage him to jump for food though, I've been throwing it mostly on the opposite end of the tank to try to have him chase it instead

I want to add that I clean the tank once a week with a gravel vac and Change at least 40% of the water, I feed them once a day, quantity wise less than they consume in 1 minute. I light the tank from 7pm to 1 am, the rest is neutral room light.
 
that tank cant hold that many fish..so i guess it might be bioload problems?
 
Looks like he might have scraped it on something, or maybe a fish bit a chunk out of him?
 
I think that pH may be a little too acidic, but im not sure

Edit: Yes that is too acidic
 
I'd re-do my Tests, Take a water sample to the LFS and let them test. I can't see how in the world your Nitrate can be 0 with a 40% water change and gravel vac once a week with that bioload. Maybe you can hold it down to 20ppm, But not 0.

What filtration system are you running? Do you have any full tank shots? Any weird spots on anything else?

And I'd have to agree with an above poster that from the pics it looks like he dashed into something. in the reflection of the one pic it looks like you have some Large stone in the tank. Possibilty he spooked and dove into it?

What do you feed him? Agree on bringing the test to the store, also is it like a hole or a lump? can't really tell from the picture
