Help!!! Unknown worms

Queens, New York!!!
I have a 75 gal. with blood parrots and clown loaches. I clean up the tank in a weekly basis. I've noticed little worms in the gravle when I changed the water but today they've multiplied. I fed the fish worms before but these ones don't look like the black worms I fed him. I don't know what to do. I need help!!!!! Picture of the worms and the full tank attached.

Its weird, I change about 30% of the water weekly, like clock work.. But I have fed them the black worms in the few weeks. Although some of the worms get into the gravel. I don't understand why I have a different worm in the tank.. When I chg the water, i do see some of the left over black worms in the bucket. But these other little things, there are hundreds of them... What do I need to do to get rid of them? I have about 12 clown loaches, but these things seem buried in my gravel....
 
they're planara they come when there is excess of waste old food etc.and they feed on it the only thing to really do is heavy water changes to get rid of them
 
No, these are not planaria nor bloodworms. They're detritus worms or annelids, harmless creatures and will appear in poorly maintained tanks.
 
i'd suggest removing most of it and then do a couple water changes, clean the removed rocks, then reinsert into the aquarium
