Hard to make out in the pic. Does look like a nematode or parasite of sort.

I appreciate the responses. It seems to make sense to quarantine and try to treat this poor guy. I will try Api General Cure since I have some on hand. unless some one else has a better idea or advice as I am open to it.
 
I would treat the whole tank that it is in. The other fish may have the internal parasite (if that's what it is).
 
It would be hard to treat the whole tank as this is a 180 with 40 gallon sump.
