What kind of filtration? A canister may spike nitrate levels if it hasn't been cleaned for a while. Yes do 30% changes and test test test. I would think that a dead fish would first raise ammonia levels.

I also vacumm substrate at least once a week.

I wouldn't have a canisterfilter on a 10 gallon tank

Do around... 50% ... then 20% .... 20% water changes consecutively... I had to do that to remove my nitrate spike
