I Have a green terror about 6in. It looked like he had like a little polyp or white dot on his head. couple of days later it turned into a hole. It looks red around the edges. my water parameters are fine. the only thing I can think that might be the problem is the feeders. its not that much. i give him feeders like twice a month as a treat. rosie red minnows to be exact. It looks like it is spreading fast. one big hole in the center of his head and one lil one right next to it. any suggestions. so far i did a water change and added salt. do i have to quarantine him?

hmm. HITH doesnt usually spread very fast. But if its holes you see around his sensory pits. i'm not sure what else it would be.
Feeders... just stop using them. Nothing but problems can come from feeding them to your fish. Some people just experience problems later than others.
The fact that it was a polyp is interesting. A picture would certainly help. Its really hard to diagnose without seeing the problem outright. Clear pics are even better

I say if you have the means to quaranteen him, then do it for sure. Untill you know what the problem is and if its contagious.
I would continue what you're doing already. Water changes no less than once a week, but twice would be better.
Aquarium Salt 1 Tbsp/5 Gallons
And I always like to use Malafix for any open wounds, ulcers, or injuries. So that could help as well.
And cut out the feeders for good unless you raise them yourself.
Stick to high quality pellets and treats a few times a week like earthworms, crickets, krill.

First thing you should do is not feed any more feeders. Get the GT quarantined and start treatment with metronidazole and continue salt to help heal the wounds.
Another thing that worked for me was, concurrent with above treatment, supplementing the fish food with vitamins and Celcon... soak pellets in this stuff first before feeding.
