id go with bad water conditions. can i ask what amount of water changes you make on the tank and how often?
when i got my 7ft tank for oscars i noticed what seemed like HITH and tried a few different things. it seemed severe and sudden it turned out that the heater i had in the tank wasnt enough and one sider was cold and the other side warm. adding another heater solved this problem. im not saying this is your problem but its worth a mention for anyone else searching the forum for HITH in the future.

In turn this is either caused by bad diet or by parasites.
The med of choice for this is metronidazole, sold in your LFS as hex-a-mit.
Remove carbon ( as stated) as many will strip out Vit K and also will strip out the meds.
Only use carbon to remove meds, it should not be a permanent part of your filtration unless you have chronicly bad water. If you do use carbon constantly you need to replace it every 10 days - 2 weeks...which gets pricey.
change 25% or more water before each treatment.
raising the temp also can help ( up to 90 F with max airation)
