Sadly, no matter what voltage the machine was at, no frankenfish.
As for me, probably like 50-60 tetras some silver dollars and a cichilid.
3 rosy red minnows, 4 zebra danios, 1 guppy, one darter, 2 Cory catfish, 1 kuhli loach, 2 dwarf gouramis, and MANY. shrimp. RIP BUMBLE THE KUHLI LOACH AND ICE THE DWARF GOURAMI. the rosy reds and the guppy and 1 zebra danios wer eaten, darter died in a water change, Cory 1 died during acclimatization, Cory 2 died of fungal infection, KUHLI and 3 danios died when the filter stopped working, GOURAMI one died fighting GOURAMI 2, (ice) and GOURAMI 2 died when his swim bladder stopped working and couldn't get air. also, the shrimp: my turtle is the god of souls to all shrimp.
This didn't result in any fish death but it's my best close call accident story.
I was filling a mesh bag with ceramic media for a HOB. Pretty simple, right?
I looked over at my desk next to my tank and saw a small white thing on it and thought "Huh, how'd that get there?" and tossed it in the bag with the rest of the small white chunks of media.
Slap the new bag into the HOB, clean everything up, etc. Sit down, satisfied with my work.
Realize I just dosed my tank with 10mg Instant Release Ambien (Zolpidem).
29g tank, the pill was about half dissolved, I did carbon and WC's, but the fish seemed fine lol whoops

Best, most honest and most accurate answer! The goal is to have them die of old age; the reality is that very, very few get to do that.
Try DC voltage; it's a game changer!

I've been keeping fish since the 50s, and killed them all, excepet the ones that are still alive now.
There are "no" accidents.
If they died of a disease. it was my own fault, because i didn't recognize it, or didn't provide the proper water parameters or conditions.
if they died because they killed each other, it was my fault, because I didn't combine them properly, or crammed them in the wrong size tank.
I "have had" some killifish that lived longer than a season, so that was longer than they should have lived, in reality
but that was negated by those cichlids that should have lived 10 years and only made it 3.
It's the nature of the hobby....we kill fish....we kill them all
There was a sayng at old my aquarium club in Milwaukee, (or maybe it was the one in Chicago).....if you haven't killed lots of fish, you're not trying hard enough.
