How Often Do You Clean or Change Filter Media?

Wow very good to know! I try not to clean the media unless it's not letting water through because I always thought I'd be killing off beneficial bacteria. I guess it's time to clean and replace some media today. It's probably filthy. Thanks for the input everyone.
