I euthanized him today with clove oil. He seemed to be suffering.
My husband and I did an autopsy, and there was nothing in there - it wasn't the substrate.

was right (and I was hesitant to believe it) - his jaw was dislocated on the left side. That must have been painful. 
So, I guess there is nothing I could have done to prevent this which is aggravating.
I'll go to the wet spot tomorrow and look at the palmas palmas and the delhezi and try again. I want another active poly with personality. And I think both species are gorgeous. But with the 3 foot dimension, even with 18" width, I don't think I can do both until I upgrade when we move in a year or so. I'd like to have both eventually.
