ID shark gills

Ok. From my observations:

1. Your water change schedule needs to be weekly at 50%, you have some crap factory fish in that tank.
2. If this is ID is having reoccurring issues, and different symptoms every time, my guess is he has a viral/bacterial or fungal infection that isn't getting cleared up and presenting new and worsening symptoms every week. Also, cloudy eyes off and on is generally indicative of water quality issues-I'm not saying your water is terrible but I feel it is playing a part somewhere in all this.
3. That looks like ich which also tells me your fish is sick as ich generally preys on fish with lower immunity levels leading me to believe he's got some type of infection elsewhere. I'd start treating the ENTIRE tank with Paraguard to cover the fungal/bacterial/ich sides of things. Maracyn as well if his cloudy/pop eye doesn't clear up.

Side Note: I'd get a liquid test kit, the strips work in a pinch but aren't reliable.
 
It has gottensso much worse... I nowsee a white worm attached to shark and white thongs on glass... please somebody help... I dont even feel well anymore this is upsetting me so much. I added salt to the tank and have been using "quICK cure" (the blue stuff)

ten to one the white things on the glass are planaria worms which means you have too much leftover food and waste in the tank. Start doing 25% water changes every day or every other day and treat as I directed above. I bet your fish start clearing up miraculously.
 
Had a similar problem with my pangasius, after the introduction of new fish, albeit without the worms. For the cloudy eye, 40% water changes daily for around 5 days and he was good as new. No medication, no salt. Get the water right and the fish will sort themselves out. And gravel vac gravel vac gravel vac! The worms i have no experience with.
