As far as I know you need to use distilled water to get that effect. Regular tap water has too many impurities.

So don't be an idiot and microwave distilled water.
 
Well water does it, too. Every time I make tea I bang the cup on the counter to knock out the bubbles.
 
This happened to my cousin. Burnt her face including her eyelids. She is alright now. No scars or anything but she definitely wasn't a happy camper.
 
Totally heat up water in the microwave all the time to use with my french press at work, have had it spew out as I was sliding it out of the microwave and luckily didn't get injured. Now im always careful about how hot I get it and always let it sit for a bit first.
 
This is why 99% of microwave foods/products direct you to "let stand" for about 30 seconds after it's done cooking before you remove it.
 
I did this one with microwaved soup, i followed all the instructions. At the end of the "cooking" i took it out to stir it and there must have been a air bubble in the soup and it exploded all over my face, luckily i was by a sink so i didnt burn myself. damn you chunky soup.

This is exactly right. With tap water there are to many impurities to superheat the water and even with distilled water it is fairly hard to do. I have microwaved distilled water before and its not a huge deal for 2 minutes I was fine, but if you start getting in the neighborhood of 5 minutes thats when it could occur. Mythbusters on discover channel actually did a thing on it if you want to look it up. Also if I remeber right the reason it explodes up and out of the cup is becuase of the extremely large temp. difference between the superheated water and the normal spoon.
