hmm... maybe time to add some small live fish mor snails and maybe a few small shrimps...
 
Once it stops going to the bathroom for a few days along with not eating, you could also think about attempting to force feed it via injection tube, thou its a bit drastic and risky....

I've talked to a biologist friend regarding the force feeding, a few days ago, because i've done it some times but only in big cichlids. And he told me that's very risky because he is very small and it's easy to make some internal damages.
 
I read through this already, sorry if I missed it, but have you tried live black worms? I would offer them as well as keep a few dozen guppies in there as well as above mentioned. Doing a salt dose won't hurt either as well as a round of Metronidazole, it indirectly encourages appetite.
