What size tank is it? I see a Payara and a Senegal at least also how long do you quarentine your feeders? If you don't quarentine your feeders thats why it has a sore...Also can you give us your water parameters IE Ammonia Nitrite Nitrate?

It's been awhile since I had my water checked. I do 25% water changes 1 to 2 times a month.

I do quarantine my feeders for observation.... But how long should I be keeping them in quarantined?
 
Yikes! Hopefully it's a bite wound but looks more like an Ulcer. Ulcer treatmeant is topical - you physically need to clean and apply meds to the open wound. I've had my Payara shank my Chalceus and Yellow Tail Cuda niether of the wounds looked like yours.
 
That amount of water changes is not nearly enough once a week of I say at least 50% keeps fish in a healthy condition...And Feeders should be quarentined AND medicated(Mela/Pimafix) for at least 30 days IMO that or breed your own feeders but IME/IMO it is better to just stick with pelleted food/frozen....And i'd agree its an Ulcer if you can seperate him for now and treat it in a seperate tank. Also in the other tank make sure water is good Nitrates under 10ppm. BTW what size tank is it?

Thanks for the advice. My tank is 55gals.... Looking to upgrade soon.

If it's an Ulcer the tea tree oil (Mela/Pimafix) will help the edges of the wound and maybe gaurd against fungus but it will not cure the ulcer.

Looks just like a nasty gash he got from running into something or fighting with that payara or whatever else he has in the tank.
