Injury or Illness?

Hello folks. Could anybody tell if this fish is just injured from the rocks in the aquarium, or is it some type of illness?

There is a small patch on one side, but a large patch on the other. Also, there is a small spot on the tail.

Looks fungal...
 
Also to add, it was not there this morning. Up close, and without my crappy camera, there does not appear to be anything other than missing scales. It does not look like any "fluff" from any fungus I have ever seen, but I cold be wrong. Also, no other fish are affected.

Qhat are the tankmates? What baffles me is both sides. Does he scratch himself against surfaces?
 
He does scratch himself sometimes. He will turn upside down and jet by rocks. He's been doing that for about a year now. Tankmates are a largemouth bass, rainbow trout, and a zimbabwe tilapia.
 
They are legal if you obtain them from a private pond owner that has a permit.

A person may possess Mozambique tilapia in a private pond subject to compliance with §57.116(d) of this title (relating to Exotic Species Transport Invoice).
 
I still think it is some kind of rash.

Have you added salt?
