Is this Columnaris or HITH?

It's HITH.

What do you usually feed this fish?
Tank size?
Water parameters?
Any other fish in the tank?
Temperature?

Thank you. It is in a 75 gallon with 4 clown loaches, 3 silver dollars and a delhezi.
Temp is normally at 75 - 80 degrees.
Water parameters are:
ph-7.4
ammonia- 0 ppm
Nitrates- 10 ppm
nitrites-0 ppm
Filtration is a FX5.

I have used pretty much every treatment I can find.
 
No meds will work. Try giving it various foods with multivitamins. I don't understand how this happened even with proper water parameters. It must have been lacking nutrition.
 
I feed it mostly hikari gold. Sometimes dry krill, and silver sides. I will try earth worms, any other suggestions would be appreciated.

South Americans like Oscars, Geos for some reason get it like cancer for reasons I think sometimes unknown? I have had some get it with over filtering & feeding the best foods & still get it!

Get Clout. I've used it numerous times to cure orphaned oscars. It works wonders just keep up on waterchanges and follow dosage
 
Twice a week water changes, add two multi-vitamin blocks to the tank. You could also soak it's food in a liquid multi-vitamin. Meds will only help with any secondary infection should one occer but will not cure the HITH.
 
I believe that lacing its food in garlic extract will help
