im sorry to say that looks like hith . check your water permeters . water change. soak your pellets in Epsom salt and feed for 5 days.
 
looks like it to me, cure is long term pristine water. nitrate never gets over 20ppm, i would also keep tank dosed with medafix and pimafix. this will not hurt your bio filter, this took a long time to acquire and takes a long time to fix or heal.
 
more than likes as most cases you thought your water was great because it was crystal clear. this can not be further from the truth. good bio filtration and mech filtration will keep your water looking pristine even as your fish die, water changes have to happen according to your tanks nitrate creep, get the kit, do the tests and find out what your tanks WC needs to be.
 
I have a sump rated for a 200 gallon tank for my tank and I do a 50% water change once a week. I also have my sump turned into a refugium and it also has a ton of bio balls in it as well. I also am running a carbon reactor which I change out the carbon onece a week as well. I just tested my water and my nitrates were really high. I don't understand how this happens though?

more filtration will not reduce nitrates, the only way to reduce nitrates is water change, nitrates are the end product of ammonia to nitrite, nitrite to nitrate. no matter how much you have for filter the numbers the same, x amount of waist will =x amount of nitrate. this is a fact, water changes need to be done to reduce nitrates as there is no other means.
 
I am thinking about doing a 30% water change every day for the rest of the week. Tomorrow I will do a 50% wc on my 125 and 2-3 wc's a week from there on out. Do you think this will the disease my pike cichlid has?

no need for every day water changes, better off doing a fin level (90%) every other day, conditioned with seachem prime , dosed for the size of the the tank not water replacing until you can get a water test test kit such as a API master Fresh water Test kit. do not get test strips.

again pristine water are what you are looking for. and with out knowing what your water perams are better to replace as much as you can until that's known. prime lasts 48 hours, hence the every other day schedule.
