Based on the amount of filtration you have, and your amount of water changes my guess is your tank is overstocked. It may not be so overstocked that you must get rid of some fish or get another tank, but you will need to up your water change schedule (as others mentioned).

One thing is be careful doing huge water changes. If the new water is not oxygenated you will suffocate your fish. This happened to me when I first started out because I was doing a huge water change after my nitrates creeped up. Ended up losing almost an entire 55g tank's worth of african cichlids - needless to say a bad day that I still remember.
 
Can't carbon be the culprit here? Seems I remember it being potentially bad for oscars.

naa, carbon can be a issue once its fully charged then it releases but HITH is long term bad water quality and bad diet. 99.99% of the time its water quality. cure is the opposite. medafix and pimafix will not cure it, they are just added to prevent secondary infections while the fish heals.
 
I only have three fish in the tank which is an 125. There is an oscar, a geophagus, and an orange pike cichlid. I wanted to have a lot of filtration because I knew these fish were messy and I wanted to make the tank as steady and less stressful on the fish.

I am sorry to ask again fish size, and what has water change schedule been, how much and how often. good filtration a great thing but does not decrease water changes needed.
 
Clean water is key. Metro has helped my fish with HITH in the past too but it's debatable if it was that or the cleaner water that made the difference.

Pothos and other plants can really help slow the nitrate creep. Check out the pothos thread on here.

I also swear by purigen. I'd replace your carbon with that. It can be regenerated and absorbs excess nitrogenous waste.
