Lathargic flowerhorn with white patches

Water perameters are 0 nitrite 0 ammonia and 40 nitrate. Tank is a 55 gallon established for a year and a half. Filtration is a fluval 406 and tank is aerated and a circulation pump is also in the tank. So the story is he was transferred from my 210 after he was getting a little beaten up to this tank about two weeks ago. He never regained his vigor and is now resting on the bottom and breathing has become harder for him. He won't eat and I haven't seen him poop for a while. He has white spots on his face and bottom of his throat area. Possible bacteria secondary infection? Columnaris? Please help any suggestions will greatly help. And I just did a 75% water change.
 
Any guesses? Also looks like he might have little black spots over his body. No way I could get a pic of it cuz they're too small.
 
Nitrates were before the big water change and I guess I'm gonna keep doing daily 25% wc or so. I added epsom salt to the tank and tried to feed him 3% soaked pellets but he won't eat. He just sits at the bottom. I just don't know what the white patches are. Don't have much experience with fish disease but I have maracyn and maracyn 2 and wondering if I should start with treatment. I was thinking some kind of bacterial infection from wounds. And if it is columnaris I want to get a jump on it before I lose this guy.
 
Man he is not getting better but not getting worse. He'll swim up a little and then sink to the bottom and just sit there for hours. I swear he has black spots a little smaller than ich size on him. Researched it and there's a black ich that is common in wild caught fish and has an intermediate host that is usually some sort of crustacean. I have no wild caught fish and no crustacean's. only thing I can think of is I have fed hikari frozen krill to him in the past. I hate having sick fish! Makes me wanna just give up cuz I can't help him. Frustrated:swear:

Believe it or not he's still alive hasn't eaten in 11 days or so. The spots ulcerated and opened up with white furry tissue around it. Treated him with maracyn and maracyn 2 but haven't seen any improvement. He'll lay completely flat on his side like he's dead and I lift him up with a pipe to at least get him on his belly. Trying to force feed with liquified nls with metro. It's hard cuz every time I think I get it in his gut he spits it out in the water. I think he's gonna die soon and it sucks.
