Some help please? My leopard cko, full grown, SHTCT, has been puking out mealworms recently. He is already de-wormed and is currently showing the basic signs of a healthy lizard, such as a clean cloaca, hungry, alert, active. What could be the apparent reason behind this?

How long have you had the animal, what are your temps, does it get fed anything else, what is the substrate, when was the last bowl movement, how is it supplied water, what else does it eat?

Why did you deworm him? Leopard geckos are allways cb and even if wc there is hardly the need to do that. That can be worse then the parasites themselfes. Wait several days before feeding again.

Tropical temps, its at least 24 degrees at night over here all year round, and definitely warmer in the day, perhaps about 27 degrees in the day. Water is supplied through a bottle cap, which has been cleaned out throughly before, bowel movement occurs most recently about two days ago, and it occurs once every two days constantly. I'm afraid its only eating mealworms.

De-worming was done years ago by its previous owner before I got it.

If it keeps puking mealworms try something easier like some wax worms. Just make sure it doesn't get spoiled with them because they're really fatty and it might start refusing everything else. Start a colony of feeder roaches, feed dusted crickets, heck for an adult a pinky every few months would be a great addition to the diet as well.

Doesn't sound like anything to be really worried about. I'm assuming you're talking about degrees C with all your temps so I suggest adding a basking spot of 33-36C to allow the animal to really heat up to digest its meals.
