Lost three fish - How to prevent in the future

I treated with Sentry Coppersafe. It said it was for Ick and other parasites. When I saw the tiny white spots on the first Bala, I assumed it was a parasite. I never thought of a fungus. What is the best way to determine the difference and make sure the tank is safe?
 
How old is your carbon? After awhile is can leach all the bad stuff back into your tank, I wouldnt even bother with carbon anymore, plus without it your gph will increase. Also do you dechlor your water first?

How long have you had this particular fish, and what do you feed it? If feeder fish are involved id say that's what the cause was!

There are some nasty strains of Velvet out there... and its like pepper covering the fish... and resembles Ick alot.. but appears more.. hazy.. My bets on that. Ive had a strain wipe out my comm tank in a few days despite anything I did ( the lfs i bought the fish from also lots almost 50% of their stock, and its a huge well respect fish store)... my fish just keeled over like flies.. its nasty.. and horrible. But it can happen to anyone. IMO let it run its course with anything living left for a month... if after a month your survivors are healthy and otherwise everything is normal add a few fish. Recycleing the tank, breaking it down ect... won't guarantee anything.. time, patience, and abit of dilligence will imo.
