This tank was setup with an already cycled cannister filter and substrate and driftwood moved from an already cycled tank.

for ick, crank the heat, 87 or better, it will get worse before better, I also agree with paragaurd, your nitrates, 40ppm out of the tap I have no answer to.

Heat has been at 85 for several days. I'll go pick up some paraguard today. Should I halt the current ich med?

the purpose for salt is its an irritant, fish make a heaver slime coat, I do not think it is needed for ICK , just heat, life cycle be sped up to much to survive. others differ.

you need the heat over 86. if can get to 89 to 90 best, This temp will not hurt your fish.

Well I'm in the middle of doing a 50% water change now soooo

The salt was added AFTER they were seen flashing
the way you posted made it seem like the loaches were flashing after the first round of treatment.
I would probably stop the current med...that just makes me nervous with the amount of sensitive fish you have in the tank. Paraguard I have used successfully in tanks with all the above mentioned fish with great results and from what I could tell no ill effect to the fish.

A pic would really help,
