the head is there just give him time. Feed him a whole bunch for the next day or two and then starve him for 2 and his head should pop. Then just let nature play its course. Don't overfeed him. Give him time and he will be sweet when he reaches 5-6".

It sounds inhumane but fish are fish, not humans. They don't need to eat every day to be healthy. It's kind of like how a snake only eats once or twice a week. They should be fasted for 2 days at a time every now and then (at least once a month) anyways to clear out their digestive system. I usually skip out on feeding my fish 1-2 days a week and all of them are very healthy.
The method does work too. It's a myth that "fish store food in their head." If that was the case then their heads would get smaller after not eating for 2 days. Every time I starve my fish their nuchal humps get huge.
LOL what is it about flowerhorns? They're so appealing, despite the fact that if I woke up to that face looking down at me I'd probably die of fright.
Updates on Jan 10,2009. I have fed it on thursday and starved it for 2 days. The head has grown a small bit. i keep do that once every 2 weeks.

I think I can see an improvement in kok size too. Did the same with my flowerhorns and one guy was so hungry he went for my knuckle during his waterchange. I think I can see a bit of improvement too on my fish.
When he gets to 6 inch+ I better watch where my hands are when I don't feed him for a day or two.
