The OP never stated the size of the fish. My statement was a full grown FH will require a larger tank, however if the FH is 6" the tank size isn't the issue at this point. I try to gather as much information as possibile and reserve judgment untill I have some facts to base my opinion on. Since I am unable to give the OP specific instructions as to how best to treat his fish I gave him some basic information to help him improve the conditions in which he is keeping it. I realize I'm being a little short with you but when people come into this section looking for help they deserve the best help we can give them based on the information available not one sentance off handed remarks to embaress them and make them not want to come forward with a problem.

My advice to you is to watch, listen and learn form some of the experianced members on this site. You'll be surprized at the information you pick up.
the only detail I have gotten is that its in a two foot tank, so I have to work with that. im eager to find out various details about their water chemistry, however as far as we know the OP might not even have access to those, and we have to work with what is given to us. so far the part about the worms intrigues me, which could probably mean some kind of parasite
 
Sounds like parasite to me..
Salt is your best bet with meds. And turn up the heat.

The worms, pending a photograph are likly planaria or detritus worms, both of wich are from lack of maintance, gravel vacuming during water changes and sufficent filter maintance. Neither of them are parasitic. However this is only speculative at this point but sounds logical from the discription.

Re read the original post there are other details in the text, not many but a couple.
