Sounds like you will need more bio. Especially if you are keeping a clean tank like you say.
If it is when you're doing you WC's with the gravel vacs it is simply a bacterial bloom.
A good way to fix this problem is to a) don't gravel vac the entire gravel bed everytime(vac one half and just get the gunk off the top on the other), or b) add another filter to house more bacteria or replace your current filtration with a one that includes more bio-filtration.
When you are doing your full vacs of the tank you are taking out alot of the good bacteria in the bed, which results in cloudy water untill it balances itself out again.
This is a good way of your tank letting you know you don't have enough bio-filtration. If you had fake plants and ornaments in your tank it will also reduce the the chances of getting a bloom.
