Need Help.. Fish Dying and dont know why?

I change 50 percent weekly.. No ammonia, No Nitrite, 40ppm Nitrate.. I check it weekly and just checked it again to be sure...
Also the fish arent gasping at the top either. Not even heavy breathing.. I have two redtails in the pond. One is fine and swimming around like he has no care in the world and the other is like this???
Well, The RTC died this morning. The other one is still doing fine the with no signs of what this one had. Did my weekly water change last night. Did a real large water change instead of the usual 50 percent. everything was zero this morning besides 10ppm of nitrate. So Im 99% positive there is no water quality problems in this pond. Some kind of infection, disease, parasite.. Im just hoping for the best. Still some amphilophus sp. not doing too well and swimming with their backs out of the water???
