Need help!

Is there anything visibly wrong?
There are no visible things wrong with him. He is just refusing food and breathing too often. And once again there was never any surface agitation since I got him and this problem started about a week ago now.
 
I would try setting the filter so there" is" surface agitation, to see if that helps.
I would also change a lot more water per week. (I change 30-40% of my tanks water every other day)
With the already high nitrates, and such a wimpy water change schedule, I believe your tank water may becoming a toxic soup, the fish is trying to escape from..

Snakeheads generally like minimal agitation , but as it grows its o2 need is going to go up, and it’s going to need more agitation to give it such.
duanes duanes also makes a great point about water quality, I’ve had many fish in the past tell me their water wasn’t clean enough and start jumping out.
